Living with a chronic condition can be challenging. You don’t have to do it alone. Meridian’s GuidedCare program connects you to care, services, and resources to support your health. For members who would like extra support, the Chronic Disease Management Program offers health coaching to help you manage your condition and build healthy habits.

Support for your health, at no cost

Meridian's free Chronic Disease Management Program offers one-on-one support to help you understand your condition, manage your health, and build healthy habits. Your health coach can help you:

  • Understand your condition and treatment plan
  • Stay on track with care, medications, and appointments
  • Watch for warning signs and know when to get help
  • Set health goals that work for you
  • Build healthy daily routines
  • Feel more confident managing your health

You can connect with your health coach by phone or through a secure messaging app and reach out whenever you need support. The program is confidential. You can stop at any time.

Health coaching may be a good fit if:

  • You are a Meridian Medicaid member
  • You have a long-term health condition
  • You would like extra support managing your health

There are a few ways to join:

  • Your doctor refers you
  • Meridian contacts you
  • You sign up yourself

If you feel sad, stressed, or overwhelmed, help is available. This program provides support but does not replace therapy or counseling.

A health coach can help you:

  • Learn to manage stress
  • Feel more in control of your mood

This program is for adults only.

Asthma and chronic obstructive pulmonary disease (COPD) are lung conditions that can make it hard to breathe. Common symptoms include:

  • Shortness of breath
  • Coughing
  • Wheezing
  • Tightness in your chest

Both conditions can be managed with the right care and treatment.

Your health coach can help you:

  • Use your inhaler the right way
  • Control breathing symptoms
  • Avoid flare-ups
  • Develop a self-management plan
  • Quit smoking, if needed

Asthma coaching is available for children and adults. COPD coaching is available for adults.

Diabetes is a condition that affects how your body uses sugar (glucose) for energy. Managing diabetes can help you feel your best and lower your risk of serious health problems. If you have diabetes, we can help you:

  • Check, monitor, and understand your blood sugar
  • Take diabetes medicines the right way
  • Exercise and eat healthy foods
  • Prevent diabetes problems
  • Quit smoking, if needed

Available for adults and children. Parents or caregivers may join too.

These programs help with daily health goals, such as:

  • Healthy eating
  • Being more active
  • Weight management
  • Quitting tobacco or vaping

Programs are available for adults and children. Tobacco support is available for teens, and pregnant members can receive tobacco cessation support.

Heart conditions affect how well your heart and blood vessels work. Without proper care, they can increase your risk of serious health problems such as heart attack or stroke. Our health coaches can help you manage:

  • High blood pressure (hypertension)
  • Coronary artery disease (CAD)
  • Congestive heart failure (CHF)
  • High cholesterol
  • Hyperlipidemia

Your health coach can help you:

  • Understand your heart medicines
  • Watch for warning signs and know when to contact your doctor or go to the emergency room
  • Make heart‑healthy food choices
  • Stay active safely
  • Learn how to monitor your own blood pressure
  • Quit smoking, if needed

These programs are for adults.

Sickle cell disease affects red blood cells and can cause pain episodes, infections, and other health concerns. Regular care and support can help you manage symptoms and improve your quality of life. Members with sickle cell disease are automatically enrolled in health coaching.

A health coach help you:

  • Manage pain and symptoms
  • Stay on track with your care
  • Avoid emergency room visits
  • Get support for your daily needs

This program is available for children and adults. Parents or caregivers can be involved.
