MomCare Connect has expecting moms covered.
At Meridian, we connect our pregnant members and their families to the care and resources they need. Our new program, MomCare Connect, offers personalized care and education. This helps moms and babies stay strong and healthy.
MomCare Connect fosters healthy beginnings for families through ongoing guidance and care. The full-service care program runs from pregnancy to the baby’s first birthday.
As an expecting mom, you’ll have your very own Meridian MomCare Advocate to support you. That “helper” will connect with you however you prefer—by phone, email, or in person. And your MomCare Advocate will provide support for a dad, life partner, or other caregiver!

Personalized support for every mom
Expecting moms at higher risk for pregnancy problems can get extra help. Our Start Smart for Your Baby offers personal support from a care manager. Members who qualify may include expecting moms with health issues like diabetes, high blood pressure, a history of miscarriage, or past premature births.
- Each pregnant member is assigned a MomCare Advocate. They will be side by side with you through your pregnancy and your baby’s first year. They will offer support when you need it.
- You can reach out to your MomCare Advocate anytime you need help. You don’t have to wait for them to call you.*
- Your MomCare Advocate is experienced and prepared to answer your questions. Credentials, training, and certification they hold include:
- A bachelor’s degree and four to six years of related experience
- [Lactation consultant certification]
- [Training]
They will:
- Get to know you and your needs.
- Provide one-on-one planning and assistance, such as making appointments with your doctor or a behavioral health provider.
- Meet you where you are
- Reach them by phone or email, or they can meet you at your home, a local coffee shop, or other location.
- Share details with dads, partners, family, or friends who support you. This includes grandparents and siblings.
- Listen to your thoughts and feelings. Support a pregnancy journey that is just for you.
- Help work through any barriers you encounter during your pregnancy and after.
Access to whole-health care, providers, and much more
Along with 1:1 help from MomCare Advocates, members also get:
- Up to $150 on a prepaid debit card and free baby gear through our Healthy Rewards program
- Submit a Notification of Pregnancy (NOP) and earn a $100 prepaid card if done in first trimester or $50 if done in the second trimester
- Complete prenatal care visits and earn $50 prepaid card for a visit in your first trimester plus your choice of baby gear (choice of car seat, cribettes, highchair, stroller, or baby shower in a box)
- Help with birth plans
- Access to a doula or midwife
- Help with resources for gender reveals and baby showers
- Healthy eating advice for pregnant moms and resources to find fresh food in your area
- Access to digital tools such as food and pregnancy apps
- Local aid and support groups
Support once your baby is born: Ready to Grow Program
After you give birth, you’ll receive continued support with our Ready to Grow program.
You’ll get:
- Ongoing help from your MomCare Advocate, including access to resources just for new moms
- Help finding a pediatrician, a doctor who specializes in caring for babies
- Help with setting up doctor appointments
- Breastfeeding help from certified lactation specialists
- Calls from your advocate to check in on your physical and mental health well-being
- Information on your baby’s needed doctor visits, including immunizations
- Guidance on how to monitor your baby’s growth and development
- $100 prepaid debit card for your provider visit up to 12 weeks after giving birth
- And, up to $230 in your baby’s first year of life for completing checkups and immunizations
