Separate Claims for Wellcare Meridian Dual Align (HMO D-SNP) and Meridian Medicare-Medicaid Plan (MMP) Claims Reminder

All members previously enrolled in our Medicare-Medicaid Plan (MMP) experienced coverage and member ID changes at the start of the calendar year. Please follow the steps below to avoid rejected claims and processing delays.

Professional services rendered in 2025 and 2026 must not be billed on the same claim.

  • 2025 services were covered under the MMP product
  • 2026 services are covered under the new Wellcare Meridian Dual Align (HMO D-SNP)
  • Verify eligibility to confirm the members' current coverage and member ID
  • Obtain new prior authorizations for services beginning January 1, 2026, or after from Wellcare by Meridian.
    • Prior authorizations issued under the MMP (prior to January 1, 2026) for dates of service ending on or after January 1 will transfer with the member’s eligibility to Wellcare By Meridian D-SNP. These authorizations will be honored and do not need to be resubmitted. The authorization number will remain the same.

To ensure accurate processing and payment, please submit separate claims for each calendar year based on the applicable product. Combining services from different years on one claim will result in claim denials or processing delays.
