New Developmental Disability Case Management Team Supports the Unique Needs of Adult Members

Meridian is proud to introduce a dedicated case management team specially trained to support the unique needs of members with developmental disabilities, ages 22 and older, as well as their providers and caregivers.
Development Disability Case Management Team (DDCMT): Overview
As children with developmental disabilities age out of services, our new case management team is here to help navigate available services and resources.
The DDCMT supports members, families, and providers in the following ways
- Personalized care and seamless coordination of services
- Guidance on insurance benefits—for the member and care team including education on support offered through Illinois’ home and community-based services waiver program for Adults with Development Disabilities
- Employment resources and job training
- Identification and collaboration with providers and community
