MERIDIAN'S BACK TO SCHOOL HEDIS® CHECKLIST

To assist our providers, we have developed a back-to-school HEDIS list. Please complete the following services during patient visits to ensure a heathy start to the school year.

Immunizations for Adolescents (IMA)

• Adolescents must have the following immunization series between their ninth and thirteenth birthdays: Meningococcal, Tdap, and HPV

• Please educate parents on the importance of the HPV vaccine starting at age 9 and have resources available

Oral Evaluation, Dental Services (OED)

• Children and adolescents under 21 years of age must have a yearly oral evaluation with a dental provider.

Weight Assessment and Counseling for Nutrition and Physical Activity for Children/Adolescents (WCC)

• Calculate each patient's BMI percentile and discuss nutrition and physical activity

Well-Care Visits (WCV)

• Schedule a well child visit each calendar year for children and adolescents ages three to 21

Childhood Immunizations Status (CIS)

• Children must have the following immunization series before their second birthday: DTap, HiB, HepB, IPV, MMR, PCV, VZW, HepA, Influenza, and Rotavirus

Well-Child Visits in the First 30 Months of Life (W30)

• Children must receive six or more visits in the first 15 months of life and an additional two or more visits between 15 and 30 months

 

All Medicaid members under 21 years of age are eligible for an Early and Periodic Screening, Diagnostic and Treatment (EPSDT) visit. These visits are key to ensuring children and adolescents receive appropriate preventive, dental, mental health, developmental, and specialty services. Visit the Meridian website to learn more.  

Meridian also offers contracted providers a P4Q program (PDF) which is a pay-for-performance incentive program. Incentives are paid after completion of all qualifying services in compliance with HEDIS 2023 guidelines.
