2025 HEDIS® Medical Record Submission Deadlines and Guidance

Timely and accurate HEDIS® reporting is essential to measure Meridian’s performance on important dimensions of care and service. New in 2025, all medical records should be submitted through Availity®. For Measurement Year (MY) 2025, medical records may also be submitted through the Meridian secure provider portal or Wellcare secure provider portal as our family of plans completes migration to Availity as our exclusive portal.
To support providers, the Quality Improvement team has issued the following additional guidance for MY 2025:
MY 2025 Key Deadlines
- October 31, 2025: Feedback deadline
Deadline to request feedback on medical records using the standard feedback tracker template
- December 1, 2025: Medical record submission deadline
Deadline to submit medical records through Availity or secure provider portals for guaranteed abstraction*
- December 2, 2025 - January 16, 2026
Medical records may be submitted during this time frame but are not guaranteed for abstraction
*Medical records submitted by email or fax are not guaranteed to be received or reviewed.
Medical Record Feedback
- Requests for feedback on medical records must be submitted on or before October 31, 2025, only through the Meridian secure provider portal
- Feedback will be provided for up to 100 medical records
- The standard feedback tracker must be used
