ZEPBOUND UPDATE

Effective September 15, 2026:

Zepbound (tirzapetide) is non-preferred for Meridian Illinois. The prior authorization criteria for the indication of obstructive sleep apnea (OSA) have been updated to include a trial and fail of Zepbound KwikPen as follows:

Initial Approval:

  1. If request is for Zepbound Pre-filled Single-Use Pen formulation, member must use Zepbound KwikPen formulation.

Continuing approval:

  1. If request is for Zepbound Pre-filled Single-Use Pen formulation, member must use Zepbound KwikPen formulation.

Zepbound and GLP-1 agonists for weight loss remain a benefit exclusion.
