Supporting Members with Substance Use Disorder – Wayspring SUD Home Program Available July 1, 2026

Meridian Medicaid Plan, in partnership with Wayspring, is pleased to announce the launch of the statewide Wayspring Substance Use Disorder (SUD) Home Program beginning July 1, 2026.

Program details
Working in conjunction with Meridian Care Management teams, Wayspring’s SUD home program provides a structured, personalized, proactive support system for individuals with SUD, holistically guiding members through treatment into early recovery to reduce harm. Eligible members are adults aged 18 and older with a diagnosis documented by claims history within a 24-month period.

Clinical overview
The program delivers an integrated, longitudinal model of care through a multidisciplinary clinical team working alongside Meridian Care Management to reduce common barriers members may experience when seeking SUD treatment. Services are delivered in person or via telehealth by licensed Wayspring providers.

Onsite visits
Wayspring will conduct onsite visits to engage and interact with Meridian members and partner with Meridian’s onsite care management staff. The Meridian behavioral health, transition of care, and care management teams will interact in tandem with Wayspring for member referrals.

Key services offered by Wayspring

  • Addiction treatment, including medication-assisted treatment
  • Behavioral health services and therapy
  • Primary care services
  • Care coordination and referrals to community and specialty providers
  • Active facilitation of social needs support, through successful connection. Examples include warm handoffs, application assistance for housing, food access, transportation, and employment
  • Ongoing engagement and follow-up to support recovery and reduce readmissions

Implementation
Wayspring partners with Meridian Care Management staff and with a contracted facility to coordinate care, assist with discharge and aftercare planning, reduce readmissions, and integrate with existing workflows. There is no separate billing for providers for the Wayspring program.
