Provider Billing Alert – Procedure Code G2211

Meridian has identified instances where procedure code G2211 is being billed for Medicaid services.

Please be advised that G2211 is not a covered or reimbursable code for Medicaid and will be denied when submitted on Medicaid claims.

G2211 is an add-on code intended for use with Medicare Evaluation and Management (E/M) services only and must be billed in accordance with Medicare guidelines. It is not payable under Medicaid benefit plans.

Providers are encouraged to review their billing practices to ensure compliance with Medicaid billing requirements. Claims submitted with non-covered codes may be denied and are not eligible for reimbursement.
