Promote Whole-Person Care and Earn Incentives by Screening Members for Social Determinants of Health

Screening members for social determinants of health (SDoH) illuminates the non-medical factors—such as housing, food insecurity, and transportation access—that influence their overall health outcomes. SDoH screening has been shown to reduce health risks and decrease the cost of care.
Effective April 1 through December 31, 2025, all contracted Meridian Medicaid and YouthCare providers may earn financial incentives for utilizing SDoH screening instruments and Z-codes.
Incentive Structure
- $0.50 for every Z-code billed on a claim
- Eligible Z-codes include all ICD-10 codes in the range of Z55-Z65; view the program flyer for additional details about eligible Z-codes
- 100 code annual limit per TIN and per NPI
- $5.00 per SDoH screening completed
- Limited to one screening per member per year
- Must include at least one screening question for housing, transportation, and food
- An eligible screening instrument with the appropriate LOINC code must be used; view the program flyer for additional details
