Meridian is Participating in the New HFS-sponsored Illinois Medicaid Preventive Care Program

As an in-network provider with Meridian Medicaid Plan, we are writing to inform you of a new Illinois Medicaid initiative sponsored by The Illinois Department of Healthcare and Family Services (HFS).

HFS has contracted with BigHeart Health to operate the Medicaid Prevention Care Education Organization (MPCEO), known as My Health. My Power. The program is designed to educate, engage, and support more than 500,000 Illinois Medicaid members by:

  • Increasing awareness of preventive health services
  • Identifying gaps in care
  • Coordinating care plans with providers
  • Connecting members to in-network primary care and preventive services

In the coming weeks and months, BigHeart Health will contact eligible members to enroll them in My Health. My Power. When care gaps are identified, the program will coordinate with in-network providers to schedule annual wellness visits and other preventive services, supporting HEDIS® measures and addressing unmet care needs.

Outreach efforts will focus on three priority regions: South Suburban Cook and Will County, Central Illinois, and Southern Illinois.

Please anticipate outreach from BigHeart Health to assist with scheduling primary care visits for enrolled members.
